R.N. Case Manager
Home Health of Western Colorado is looking for a full-time RN Case Manager to join our Team!
Qualified candidates will have previous Home Health care experience and will be instrumental in planning and providing direct care to clients as assigned.
Must have the ability to travel between Montrose and Delta counties as needed. Some weekends and On-Call duties would be included requirements with this position.
